Improving care for people with long term conditions in Harpurhey

Long-term health conditions cannot be cured, but they can often be managed or controlled by some form of medical treatment or by some other form of intervention.  

About a quarter of people in Harpurhey are thought to be living with a long-term health condition such as: Hypertension, Obesity, Asthma, Diabetes, Coronary heart disease, Cardiovascular disease, COPD, Chronic kidney disease, Hypothyroidism, Stroke/TIA, Cancer, Epilepsy, Dementia, Mental  Health (including   depression   and   anxiety) - or some other long term condition.
